prepare your bankruptcy, we will need the following documents as applicable
to your case: |

Income Tax Returns, individual and business, including Schedules
and W-2's, for 2007, 2006 and 2005.
- Mortgage,
deed, or promissory note on your real estate property, most
recent proof of homestead exemption, copy of payment book and
all insurance binders and riders on real estate and personally,
including homeowner's insurance policiesfor the current year
and last two previous years.

On all automobiles, motorcycles, and boats, registration, title,
document evidencing proof of insurance and document showing
original purchase price. If secured or leased, security agreement
and payoff letter which must be requested from lien holders.
Proof of full coverage on all vehicles and vessels.
- All
insurance policies and annuities, IRA's, 401K and pension plans.
- Pay
stubs for the last four pay periods as of filing date of petition.

All credit card statements for one-year period immediately prior
to bankruptcy if available.

Documents pertaining to any lawsuit, foreclosures, garnishments
or repossessions.

Copies of any correspondence from the Internal Revenue Service
or other taxing authority.
